2026-27 Fiscal Year Applications are Now Open

To be eligible for financial assistance you must be an active member and services are rendered during active membership. 

  • 2026-27 Limits have been reduced.
  • Technology is no longer covered.
  • $500 Maximum for Recreation or Therapy
NOTE: Please only submit ONE application. You may attach up to 10 receipts/invoices in this application.
